  Out Patient Service :  
 
   
Through our out-patient services, we provide practical physical and emotional support for patients with limited life expectancy. We help to ensure that physical, social, emotional and spiritual needs are met with love, acceptance and dignity.

We provide specially trained staff (our team is made up of a doctor, a nurse, a social worker or counsellor and various volunteers), who work closely with the patient's primary care physician and family to oversee care and treatment.

Our focus is on controlling pain and other distressing symptoms connected with end-stage disease. This includes providing medications, dressing ulcers, inserting feeding tubes, administering subcutaneous fluids through an IV, and conducting Ascitis tappings, bladder washes, and catheterization. Sponge baths, perineum care and eye and oral care also form an integral part of the routine procedures we do to prevent further infections and to make the patient as comfortable as possible.

Our services:
* Provide patients with medical equipment such as hospital cots, water beds, wheel chairs, bed pans, urinals and commodes
* Train volunteers, families and patients in basic nursing care and infection control
* Give advice on nutritional requirements
* Allow patients to ventilate feelings and emotions; besides contributing to our patients' mental health, this helps us identify patient and family concerns
* Equip the patient to regain maximum independence and thereby meet self-care requisites
* Empower the patient and the family by helping them make informed choices and find strength within themselves

In addition to the services offered above, we provide Home-based Care that enables people with chronic illnesses to be cared for at home
 
  We offer the following:  
 
* Basic nursing care, good symptomatic control and social and emotional support
* Counseling and bereavement support to the family and patient based on first-hand observation of existing stressors
* Mobilization of those surrounding the patient to provide support
* Promotion of acceptance of people with cancer and HIV (still socially stigmatized diseases)
* Enhancement of the quality of life for as long as life lasts

  Bereavement Befriending:  
 
The main task in bereavement support is to help any suffering person progress through a healthy course of grieving. This is done through sensitive listening and empathetic support.

We offer the following:
* Bereavement support groups
* Active follow-up for the relatives of those who have died
* Follow-up contact to address clinical questions which may be initiated by clinicians / primary care providers within 2-4 weeks of death
* Memorial services for staff and families
* One-on-one bereavement support for families

 
  Referrals :  
 
Referrals can be made by anyone including Doctors, Social Workers, Volunteers, and Relatives of those who were under our care.
